Are you receiving medical care outside the VA?
In addition to the VA, do you receive health care from the Department of Defense (DoD) or other local Utah health care facilities? If so, you are eligible to participate in the Virtual Lifetime Electronic Record (VLER) Health Program. What is VLER? Virtual Lifetime Electronic Record (VLER) Health is a new program that shares select parts of a Veterans’ medical record electronically, safely, and privately with other approved health care facilities that are members of a secure Internet network known as the Nationwide Health Information Network (NwHIN). The VLER Health program pioneers the exchange of information between different health care systems. It is a very important step into the future of health care.
Benefits to Veterans who participate in the VLER Health Program Before VLER Health, when a VA doctor needed medical records from outside the VA, it often took weeks of waiting for the paper records to arrive. VLER Health does the same thing in just seconds. Some of the information, for example medications and allergies, may be critical to patient care and safety. Because doctors will have more information readily available, it will improve the quality of care that they provide to our Veterans. Participation is voluntary and free. How to sign up? What if I join and later change my mind? If you change your mind, you can visit your local VA health care facility’s Release of Information Office or download and complete the VA Revocation Form (VA Form 10-0484). You may then mail or fax this form to the address and number above. Do I have to sign up for both the CHIE and VLER Health? ** The following information is being provided for your convenience only. It is important to note that VHA is not responsible for enrolling Veterans into the CHIE for UHIN or its affiliates. If you would like your private health care givers to share information with the VA, you can also sign up with the CHIE program.
