Virtual Lifetime Electronic Record (VLER) Health Program
Are you receiving medical care outside VA?
In addition to VA, do you receive healthcare from the Department of Defense (DoD) or other local Utah healthcare facilities? If so, you are eligible to participate in the Virtual Lifetime Electronic Record (VLER) Health Program. What is VLER? Virtual Lifetime Electronic Record (VLER) Health is a program that shares select portions of a Veterans’ medical record electronically, safely, and privately with other approved healthcare facilities that are members of a secure Internet network known as eHealth Exchange. VLER Health program pioneers the exchange of information between different healthcare systems. It is a very important step into the future of healthcare.
Benefits to Veterans who participate in the VLER Health Program Before VLER Health, when a VA doctor needed medical records from a Community Provider outside VA, it often took weeks of waiting for the paper records to arrive. VLER Health does the same thing in just seconds. Some of the information, for example current lists of medications and allergies, may be critical to patient care and safety. Because participating doctors have more information readily available, the quality of care that they provide to our Veterans is often enhanced. Participation on the part of any Veteran is voluntary and free. How to sign up? Signing up is fast and easy. You may sign up online, through the mail, by fax, or in person. Online: Visit https://www.ebenefits.va.gov and complete the following steps: Under the MANAGE HEALTH, click to select “SHARE YOUR VA MEDICAL RECORDS.”
PLEASE DO NOT EMAIL COMPLETED FORMS TO THE VAMC By Mail: Download and print the VA Authorization (VA Form 10-0485) from our internet site. Fill out the form. By Fax: Fax the VA Authorization to: (801)584-2518. In Person: Visit the Release of Information Office (ROI) or Library at the VA Salt Lake City Health Care System. Fill out the available VA Authorization (VA Form 10-0485) and turn it in to ROI or Library staff. What if I join and later change my mind? If you change your mind, you can visit your local VA healthcare facility’s Release of Information Office or download and complete the VA Revocation Form (VA Form 10-0484). You may then mail or fax this form to the address and number above.
