Shared Care Planning
Leading progressive, multi-year ORH funded project to explore the functional and technical requirements of an electronic health record GUI with a medication reconciliation focus. These requirements are being built to facilitate a new kind of work flow and communication that is currently unavailable. This project utilizes a secure web system to house a shared care plan that includes notification of providers when updates are made to the plan, and can be controlled on a “need-to-know” basis.

Dual Use: Exploring care coordination data from Veterans referred for home health, including prevalence of prescriber dual use and prevalence of potentially severe drug-drug and drug-condition interactions resulting from dual use.

Caregivers, Dementia/ Stroke
Involved in cordination of care and decision support projects to improve dementia care management.

Patient Navigation/ Outreach/ Diversity
Exploring barriers, challenges and opportunities to the patient experience, with a focus on Veterans.

Partnerships/ Community Resource Building
Expanding partnerships through ADRC project efforts to provide training and support to Aging and Disability Resource Centers that enables them to provide more effective options counseling to Veterans and their families, with the additional benefit of linking key VA contacts with community resources to provide improved coordination of resources.

Building connections through ongoing project efforts with the Utah Health Information Exchange (UHIN) and the Virtual Lifetime Electronic Records (VLER) team to exchange secure email between VA and home health care providers. We have now successfully exchanged secure emails between VA and home health care providers, as well as, developed guidelines for process implementation for multiple users. Current project efforts are bringing together interdisciplinary teams to build a GUI for medication reconciliation, as well as, developing core requirements for electronic shared care planning.

Rural Health COIN: Developing a portfolio of interconnected research projects that further research into Rural Health challenges and needs, including mentoring new investigators and aiding/ increasing ORH proposal submissions.
